Well, seeing all the parts laid out helps prioritize the options because realistically I won't be able to get all the ones I want.

I'm thinking you 3D print two parts that snap around the head, so you don't have to hold it in place - just rotate it, so the face holes are in the front.
It's a smooth surface so the fit would have to be tight enough not to move while paint was applied. It might be better having a rubberband holding it on. I was thinking bumps to fit into the slots on the side of the head but do all heads have those?

I tried the first approach, but couldn't get a smooth enough one, and painting it (with a permanent marker) was a mess :(   

I would buy one that worked, yes...
Permanent marker ink isn't very good, it doesn't go on evenly and smudges.

There are companies that make custom rubber stamps for business use. For a face you'd need a circular stamp and a crescent stamp and then find some ink that works on plastic.

Keep in mind the super4 and spirit weirdos have different heads...

I know the permanent marked is not a good solution, but it was the best. I am not good at this. I never managed to make a mouth (yes, just a freaking mouth!) on that lame mouthless super 4 head :(

being able to do the face in the proper place (even giving up the eyes that I generally really like) would be a great improvement, especially to the freak that look like idiots. Also would make those asterix heads actually usable.
